Patient Care Coordinator (After‑Hours | Remote)** Work Schedule:** **

Monday to Friday,  11:00 PM - 7 :00 Am Kenya Time****

(This aligns with late evening to early morning hours in Kenya)

Job Type:** Remote | Full‑Time** About the Role** We are seeking a reliable and compassionate Patient Care Coordinator to support after‑hours operations for a healthcare/home care organization. This role ensures uninterrupted patient care by managing incoming calls, handling urgent needs, and coordinating last‑minute schedule adjustments during evening and overnight hours.**

The ideal candidate is calm under pressure, highly organized, and capable of working independently in a fast‑paced remote environment.

Required Skills & Qualifications**

  • Excellent communication and problem‑solving skills.**

  • Strong organizational skills with the ability to multitask.

  • Comfortable working independently in a remote setting.

  • Professional, calm, and reliable when handling urgent situations.

  • Stable internet connection and ability to work evening/night hours consistently.

Preferred (Not Mandatory)**

  • Experience in home care, healthcare, or a related field.

  • Familiarity with caregiver coordination or schedule adjustments.

  • Experience with documentation and compliance tracking.

  • Ability to make quick decisions under pressure.

Key Responsibilities****Call Management & After‑Hours Support

  • Answer incoming calls from clients, caregivers, and referral sources.**

  • Respond to after‑hours messages routed through the answering service.

  • Triage concerns and escalate urgent matters when appropriate.

  • Maintain clear, professional, and timely communication.

Schedule Adjustments (Not Full Scheduling)**

  • Manage last‑minute caregiver call‑outs and shift changes.**

  • Adjust existing schedules to maintain uninterrupted coverage.

  • Notify caregivers and clients promptly of schedule changes.

  • Support continuity of care during evening and overnight hours.

Administrative Support**

  • Update caregiver records, including certifications, compliance items, and expiration dates.**

  • Track training records and required documentation.

  • Assist with onboarding and general administrative support as needed.

  • Ensure all systems, records, and logs are accurate and up to date.

Caregiver & Client Coordination**

  • Serve as the main point of contact for caregivers working night shifts.

  • Provide real‑time problem‑solving and operational support.

  • Promote a positive experience for both caregivers and clients during after‑hours operations.
